Several
years ago a more conservative alternative to hysterectomy was developed
for women whose only complaint was heavy periods. After confirming
that the patient has no evidence of cancer or pre-cancer changes,
the endometrium (or inner lining of the uterus, which is shed with
each menstrual period) is destroyed in the operating room with a
procedure called endometrial ablation.
In
the beginning, ND:Yag laser was used for this purpose; later cautery
with a “rollerball” (much like using a lawnmower to roll over the
endometrium a row at a time) was developed. With safety as the major
focus, other methods of ablation have been developed; other forms
of energy have been used to destroy the endometrium. Microwaves,
heated water, and electricity (cautery), among others have proven
to be easier and safer to use.
About
five years ago, the doctors at Tyler Obstetrics & Gynecology
introduced Novasure® to Tyler. Both hospitals purchased the
equipment and to date over 400 Novasure® ablations have been
done in our community. Most recently, selected patients have been
offered the procedure as an office surgery, done with sedation rather
than general anesthesia. This is easier for patients and doctors
as well as reducing cost. The procedure takes less than 5 minutes
to perform; recovery is less than a day.
Novasure®
is done only in patients whose primary difficulty is heavy menstrual
periods. It cannot take the place of hysterectomy in patients with
other problems or whose bleeding is due to large fibroids, cancer,
hyperplasia (pre-cancer) and several other problems. It should not
be performed in anyone who desires future childbearing; it is often
done in conjunction with tubal ligation to ensure that a future
pregnancy is avoided.
Women
undergoing Novasure® can expect some cramping on the day of
the procedure, but pain is usually gone within 24 hours. Most activities
can be resumed at that time. Except for a discharge that may last
three or four weeks, that is the extent of the recovery.
90
to 95% of patients are pleased with the results and require no further
therapy. 5 – 10% of women may still require other treatment to control
heavy bleeding. 40% of women following Novasure® ablation will
have no further menstrual bleeding.
